Attn: [Name of Company] Copyright Infringement Notification

Dear Sir/Madame:

Re: Copyright Infringement Notification

_________________________________________________________________________

It has come to my attention that there are works on your website, [Company Name] <website address> (the “Website”), that infringe upon my copyright as the law applies in Canada, as described in the attached list (the “Copyrighted Works”).

The Copyrighted Works include [description of Copyrighted Works on Website]. I do not consent to their presence on the Website. As such, I am requesting that you immediately remove the Copyrighted Works from the Website.

I have a good faith belief that use of the Copyrighted Works is not authorized by the copyright owner (or a third party who is legally entitled to do so on behalf of the copyright owner) and is not otherwise permitted by law. I swear that the information contained in this notification is accurate and that I have an exclusive right in law to bring infringement proceedings with respect to its use.

Yours truly,
